
I, as an employee of Indiana University Purdue University Fort Wayne understand my responsibility of trust and agree to adhere to and perform my job utilizing the security procedures of the University as stated below:
- I agree to comply with the provisions of the Family Educational Rights and Privacy Act of 1974 (FERPA) and University policies pertaining thereto in my handling of the student information to which I have access. I understand all student data is confidential and the release of other than "public" information about a student to any person(s) or organization(s) outside the University without WRITTEN consent of the student is strictly prohibited by federal and state laws and University policy. The responsibility for the release or discussion of data is assigned to specific individuals in the Registrar's office.
- I understand that I am to restrict my retrieval and other computing activities to information I have been specifically permitted to access. This access to information is based on a legitimate "need to know" and directly related to my assigned duties using only the functions and utilities which I have been authorized and trained to use.
- I understand that any sign-on or password instructions issued are for my exclusive use and are not to be shared with or delegated to others. I further agree to maintain procedures within my office which safeguard the confidentiality of student data. This includes not leaving my terminal unattended while it is logged onto the SIS database and not authorizing another person to access data using my password or to sign-on.
- I will report to my supervisor any security violation as soon as I become aware of it.
- I understand that my continued access to the SIS database depends upon my compliance with the procedures and FERPA policies outlined herein. Failure to abide by this agreement will result in my access being discontinued and the possibility of additional disciplinary action.
